Overview

Coordinates team based care to provide health services to individuals, families and/or their communities through effective partnerships with patients, their caregivers and their physicians. Facilitates a shared goal model within and across settings to achieve coordinated high quality care that is patient/family centered.

Responsibilities

• Provide complete initial assessment, which includes determining diagnosis, creating a comprehensive treatment plan and identifying a positive continuum of care.
• Work with patients to provide optimum physical and mental health through utilizing individual therapy, group therapy, family therapy and case management services. 
• Crisis management and help coordinate services in emergent situations.
• Meet with families, medical providers and other interested parties in order to exchange necessary information during the treatment process and which can help coordinate a positive continuum of care.
• Develop, implement and coordinate mental health services throughout the HCMH community.
• Plan, organize and lead structured programs of counseling, work, study, recreation and social activities for clients. 
• Develop policy and procedure for mental health services in the HCMH system.
• Provide community education and market programs offered with HCMH.
• Engage in dialogue with other mental health services in the state such as Community Hospitals.
• Make reports to APS or DCS when appropriate.

Qualifications

• Follows established hospital policies and procedures.
• Current licensure required (LMHCA, LMHC, LSW, LCSW).
• Must obtain full licensure with 2.5 years of hire date. Temporary permits are not a substitution.
• Demonstrates evidence of essential leadership, communication, education, collaboration and counseling skills. 
• Proficient in communication technologies. 
• Previous experience with I.T. systems and data reports. 
• Knowledge and sensitivity to communication strategies, accessibility and ability to identify appropriate communication strategies when required.
